Harmony Health’s technology is coupled with a multifactorial medical assessment aimed at holistically looking at multiple areas of fall risk exposure.
Harmony Health works with some of the largest pharmacy benefit management (PBM) companies in the country and develops a more in-depth review of your prescription history. Armed with this data and coupled with insight from the American Geriatrics Society, Harmony Health creates and completes an accurate medication reconciliation.
It’s very important to conduct environmental assessments within the home. Understanding potential areas of concern and quickly mitigating those risks is critical. Harmony Health uses a technology-driven process that works hand in hand with human intervention.
Harmony Health generates actionable content. The goal is to create recommendations of positive corrections for both patient and provider to prevent falls. The information is routed to the patient while also being sent electronically to the patient’s medical record with the primary care physician.
